Vision Plan Options to Fit Your Needs: We understand that everyone’s vision needs are different, so we offer two great plans.

MODA Vision provides a rich benefit that maximizes your allowance for contacts or glasses.
Our new VSP option features a large nationwide network and makes using your benefits easier when you visit an in-network provider.

Choose the plan that works best for you and your family. Plan year: January 1, 2026 –December 31, 2026.

Moda Vision:

One vision exam per calendar year at no cost.
$450 allowance for eyeglasses or contact lenses.
- If costs exceed $450, you pay the difference.
Visit any licensed eye care provider and still receive the full benefit.
- Includes popular online retailers for frames and contacts.
Most providers bill Moda directly—just show your ID card.
If your provider doesn’t bill insurance directly, you can submit a claim for reimbursement.

The out of network coverage is the same as in network. With MODA, you can go to any licensed vision provider!

Tip: Maximize your $450 hardware benefit by shopping online at retailers like Warby Parker and Zenni, helping you bypass the steep mark-ups typically found at eye clinics. Be prepared to pay upfront and submit to Moda for reimbursement.

VISION SERVICE PLAN (VSP)

One WellVision® Exam per calendar year, fully covered.
$200 allowance for glasses or contact lenses.
- Frames covered up to $200.
- Basic lenses included at no cost.
- Premium/custom progressive lenses available for an additional fee.
If choosing contacts:
- $60 copayment for contact lens fitting and evaluation.
Save 20% on extra glasses or sunglasses (including lens enhancements) within 12 months of your exam.
